Follow these steps to ensure your athlete can participate:
Step 1: Register and complete ALL forms at: https://salina-ks.finalforms.com
Step 2: Print the “KSHSAA PPE Physical” and take it to your doctor for completion and signatures.
Please remember that you must complete all district forms, including a sports physical before practice starts.
